    Short Description:

    In recent years, a large number of Clinical Practical Guides (GPC) have been developed around the world, containing recommendations on diagnostic procedures, management of the disease, treatments based on empirical evidence for certain chronic diseases and self care advices for patients. These guides are guidelines and procedures for doctors which, particularly in primary care, can’t be experts in all diseases.

    To improve the quality, security and problem-solving ability of primary health care, Atención Primaria Digital aims to provide doctors and patients with ICT tools to facilitate the tasks of both in the course of the disease.

    On one hand, a dedicated ICT tool based on the GPC (Clinical Practical Guides) will be created, allowing the primary care doctor to guarantee that, for each patient and disease, the most appropriate treatments based on the available empirical evidence are being applied. On the other hand, an ICT accessible tool through Internet will be created, allowing the patients of chronic diseases to increase their active participation in the care and the treatment of their disease. In addition, they will improve its communication channel with medical and health system resources.

    Digital Primary Care project will develop an ICT platform to achieve a Primary Care more decisive and efficient with patients of chronic diseases, facilitating the tasks that doctors and patients carried out during the disease. The result will be a service of higher quality, greater security and lower cost.

    The proposed solution aims to facilitate doctor’s decision making and to promote the active participation of patients, improving their attitude towards the disease. The developed platform will be introduced in a pilot group of primary care centers (CAP) of the Institut Català de la Salut (ICS) and will be integrated with the Electronic Medical Register (RME). The GPCs aims a health service based on the best scientific evidence. For the healthcare professional they are a tool to improve the quality of their activity, which can reduce unwarranted variability in clinical practice, optimizing resources and moving towards better healthcare results.

    As result of the project, a platform will be developed to support the treatment of four chronic diseases: Diabetes, Arterial high pressure, Chronic Obstructive Pulmonary Disease (COPD) and Hypercholesterolemia. This project will allow to generate a support platform for diagnosis, treatment and monitoring of these diseases by the primary healthcare professional as well as the monitoring of the disease from the patient from their own home or anyplace.

    It is hoped a substantial improvement in the quality of life of affected by these diseases, assuring a treatment that include the latest developments in medicine, reflected in the GPC. In addition, the patients will have the necessary information to improve their autonomy in the disease management, with detailed information about alerts and treatments through Internet and mobile phone.

    This will also help to reduce visits to the doctor's, impacting directly on the cost of the treatment for the health system and the productive economy.

    To promote this participation, it is necessary to provide the patient with electronic access ICT tools to consult its personal medical history, information about its chronic diseases, receive alerts and reminders to ensure the correct monitoring of prescribed treatments and to report dates about the evolution of its illness.

    Objective:

    The objective of this project is to improve public health service of primary care to increase the quality, security and response capacity with citizens with chronic diseases, which represent approximately 25% of the population.

    To achieve this, it will create an ICT platform that will assist both doctors and patients, in monitoring chronic diseases. Doctors provide expert support for making decisions based on the latest scientific evidence and citizens will have access to written information about your health problem, consult clinical variables, receive reminders of treatments or visits, and provide new data relevant to the doctor.

    Barcelona Digital's activity
    The main tasks are focused in the design of the web platform for the patients providing them a multimodal web access. This platform implements the functionalities of queries to the healthcare system along with an alert and reminder engine.

    As a final result this platform allow citizens to increase their active participation in the care and treatment of diabetes type II with the following features:

    • Find information about the disease and its treatment

    • Check results of analytical tests related to the pathology

    • Receive and view the protocol messages disease monitoring
